HPI: An 11-year-old Afro-American girl presented at the office complaining of vaginal
itchiness.
Onset The client reported that the problem began three days ago.
Location She reported that the itchiness is in the vagina.
Duration She noted that the feeling is constant.
Characteristics She noted that the feeling is marked with watery vaginal discharge,
vaginal rash, vaginal pain and soreness, redness, and uvula swelling.
Aggravating factors She reported that the problem is aggravated by coitus and
micturition.
Relieving factors She reported being relieved by not going to the bathroom.
Treatment She reported taking no medication.
Allergies: The client reports no known allergies.
Current Perception of Health: Her present discernment of health is positive.
