Root-Cause Analysis
Catheter-associated urinary tract infections (CAUTIs) continue to be one of the most
common heath careassociated infections that occur at the MCHS Hospital. The prevalence of
CAUTIs in the MCHS Hospital is slightly higher then past years. This essay will address
CAUTIs by describing the problem, analyzing the events that lead to the problem, and will
discuss an improvement plan to reduce and prevent the occurrence of CAUTIs hospital wide.
Event Description
Since the beginning of the year the Infection Prevention and Control team detected an
increase in the use of indwelling catheters throughout the hospital, and determined that this was
directly related to the increase in urinary tract infections that occurred during the patient’s
hospitalization. MCHS Hospital has 2 times the greater use of indwelling catheters when
compared to the national average. Since January 1, 2019 there has been 2 CAUTI incidents have
been reported. The use of an indwelling catheter was the direct cause of patients being diagnosed
with a urinary tract infection after the indwelling catheter was placed during their hospitalization.
The catheter associated urinary tract infection lead to an extended hospital stay, increase hospital
expenses, and increase morbidity and mortality for the patients.
Findings and Root Cause
The main problem that was identified was lack of efficiency due to multiple practice
related issues. Lack of proper handwashing, a break in the sterile field, improper catheter
maintenance, prolonged use, and contaminated supplies when inserting an indwelling catheter
have all been associated with catheter associated urinary tract infections (Johnson, 2018).
Insertions of an indwelling catheter is a sterile procedure done at the patient’s bed side, and
should only be used when medical indications support the use.
