Problem 1
From the calculated risk priority number (RPN), the number of infections, and the
number of cases, the type of surgery that illustrates high risk is general surgery followed by
Orthopedic surgery, plastic, thoracic and oral surgery. Nonetheless, neurosurgery represents the
least risk based on the determined RPN. Therefore, in addressing the consequent risks, the
management should first examine the general surgery while neurosurgery should be examined
last because the type of surgery has the least RPN. Besides, the maximum possible risk score
from this analysis is 200 points involving General and Orthopedic surgeries. The study shows
that medical facilities should first address general and orthopedic surgeries before addressing the
other forms of surgeries that present minimal RPN.
Problem 2
The incidence rate change improved the worklist with the Thoracic surgery depicting the
highest priority in addressing the issue, followed by the Neuro Surgery, which in the other
problem has the least risk. On the other hand, general surgery depicts the least risk in this
scenario, revealing the least issue that the director should address, compared to the previous
RPN, which showed the riskiest event. In this case, the maximum possible risk is 22.5 points.
Problem 3
Changing the infection rate to 1.0 % and severity to 2.0 for thoracic surgery influenced the
priority list neuro surgery becoming the primary target for improvement in this case, with the
highest RPN= 12 points. Thoracic surgery, in this case, moves the fourth primary target for
improvement in the priority risk following the change reducing the infection rate and the
severity.
