Part 1: Intervention Development
Brochure
Intervention
Description
Objective
Provision of
training on the
use of home-
based blood
pressure cuff
Healthcare
professionals
collaborate with
community health
workers (CHW) to
conduct a home visit
to deliver and offer
training on the use of
home-based blood
pressure monitor.
Reinforce education
on blood pressure
monitoring and
medication adherence.
This intervention ensures that the
patient has self/tele-monitoring
tools and is knowledgeable on
their use to measure pressure at
home and send it to the clinic.
Dietary Approach to
Stop Hypertension
(DASH)
Hypertensive
individuals observe a
DASH eating pattern
that includes a diet
rich in fruits, low-fat
dairy, whole grains,
vegetables, and total
fat content.
DASH lowers blood pressure as
it is important in nutrients and
fiber.
High-potassium and
low-sodium salt
Hypertensive patients
will receive salt
substitutes such as
calcium and
magnesium sulfate to
be used in cooking.
High amount of potassium
through food intake helps the
body lose more sodium via
urine.
Physical exercises
Aerobic exercises such
as treadmill,
swimming, walking,
jogging and bicycle
training for one hour
per day.
Isometric training-
sustained contraction
against immovable
load for 3 days per
week.
Physical activities help in
reductions in both systolic and
diastolic blood pressure.
