After you have received prior approval from the instructor and an introductory email from the practice site coordinator, you can then make an appointment to interview the public healthcare provider you identified at the agency.Students must receive approval before doing their site visit. Students who do a site visit at an unapproved site will receive a grade of zero (0) on the assignment.The interviews will be scheduled by the group with the agency via Zoom. Email communications will not be accepted for this assignment. Be sure to bring the information you obtained during your Windshield Survey. After you have conducted your interview, you will use the information from the windshield survey and agency visit, along with the research in the literature, and develop a plan to address the need identified during the interview.When developing your community health promotion plan, be sure to consider how this plan can be implemented and evaluated.Your plan should identify the following:Identified community need: description of the need and the group most affected by this need.
