Discuss the multi-dimensional role of the community health nurse in risk reduction and health promotionDuring week one you submitted information regarding an agency that you would like to conduct an interview. After you have received prior approval from the instructor and an introductory email from the practice site coordinator, you can then make an appointment to interview the public healthcare provider you identified at the agency.Students must receive approval before doing their site visit. Students who do a site visit at an unapproved site will receive a grade of zero (0) on the assignment. The interviews will be scheduled by the group with the agency via Zoom. Email communications will not be accepted for this assignment. Students who submit an email communication will receive a grade of zero (0) on this assignment. Please remember, for this assignment you are only doing an interview, you will not be shadowing the public healthcare provider or working with patients.During this interview, you will identify a need that you will address in your Community Health Promotion Plan Proposal. Be sure to bring the information you obtained during your Windshield Survey. This will come in handy to help identify a need for which you will write your Community Health Promotion Plan Proposal. The interview should include questions such as:How do the members of the agency work together as an interdisciplinary team? Are there any community partnerships with other agencies? What is the most important community health need, and how has the agency addressed this need?Use the Public Health Agency Data form to assist you in obtaining information during your site visit. The form is found here and in the Course Resources section of the classroom. Keep this form till the end of the class. The information that you enter on this form will be needed for the agency evaluation at the end of the course. After you have conducted your interview, you will use the information from the windshield survey and agency visit, along with the research in the literature, and develop a plan to address the need identified during the interview. When developing your community health promotion plan, be sure to consider how this plan can be implemented and evaluated.Your plan should identify the following:Identified community need: description of the need and the group most affected by this need. Use data collected from the windshield survey and agency interview.Epidemiology of the community need: statistical data supporting the choice (can do as a table) You can find this data by accessing such sites as Healthy People 2020, CDC, US Census Bureau, Vital Statistics; these sites were presented in week 2 and 3 content areas.Literature Review: Conduct a complete literature review on the topic and state your recommendations based on the findings (what types of programs are successful). Summarize the findings from the literature that provide evidence on how to address the community need (do not list each article individually, synthesize the findings). State the research data which supports your proposal (based on examples of the studies that you read).Recommendations: Based on the information from the data you collected, and the identified community resources, and the findings from the literature review, what plan would you propose for implementing a community plan of care? The paper should be no longer than seven (7) pages (excluding title page and reference list) and be in APA format.
