SAMPLE LIVING WILL FOR: RAMI ALBAKAMI

I, Rami Albakami, want that everyone who cares for and about m to know that when in the event

that I am unable to communicate what kind of healthcare it is that I want, the following will

apply.

1. I want and expect that medical-team/doctors would take all those measures, which are

necessary to recover my health. These measures include sanctioned medical operating

procedures, which have yielded results in past. In case that my health declines to the

point, where it cannot be recovered to an acceptable point, I urge/recommend/direct not

to take further medical measures.

▪ The form of life or its quality, which I don’t consider being acceptable or which is

undesired is 1) prolonged state of unconsciousness, 2) Not in position to

recognize friends or family members and 3) I become totally dependent on others.

▪ I urge/request/direct the medical team/doctors to take best medical measures,

which could prove productive in my case; facilitate me in recovering my health.

These measures include surgery, organ transplants, use of artificial organs or

other medical measures that are legal. Before taking all these measures, my family

must be consulted, especially when I am not conscious.

2. If the medical team fails to recover or restore my health, to that point which is acceptable

to me, I shall not be fed by tubes.

3. When I am about to die or when I am near death, I desire that all my friends and family

members should be called. In addition, I do not want to be taken away from the

point/position/place where I am at that particular time.
