On the basis of the information presented thus far, how concerned are
you about Ramon’s elevated blood sugar level? What in his history
indicates he is at risk for DM?
On the basis of past medical history and present complaints of Ramon, a value of 166 mg/dl was
obtained on a finger stick blood sugar test, which was communicated to him. He shows little
concern about it, although he has a positive family history of diabetes in mother. Furthermore, he
also gives a history of polydipsia and polyuria, both being characteristic features of uncontrolled
diabetes mellitus.
As a result, his near fasting blood sugar level of 166 mg/dl is of concern due to the following
factors:
i. Past family history of diabetes mellitus in mother
ii. Increased thirst (polydipsia)
iii. Increased frequency of urination (polyuria)
iv. History of fall at night, when it is dark. This warrants retinoscopy of Ramon to see
any changes brought about in the macula of the eye as a result of long-standing
uncontrolled diabetes mellitus
v. Comorbidities like hypertension, ischemic heart disease episodes in the past and
atherosclerosis.
