MINUTES OF MEETING FOR THE INSPECTION OF PROTOTYPES

OF ALS & BLS AMBULANCES

Reference to Office Order No. SO (P-III) 10-2/2015 dated

23.02.2016, Inspection of prototype of Basic Life Support

(BLS) Ambulance & Advance Life Support (ALS) Ambulance

was held at M/s Toyota Ravi, Multan Road, Lahore dated

03.03.2016. During the inspection of BLS following

distinctions were found;

Reference to Office Order No. SO (P-III) 10-2/2015 dated

28.03.2016, Inspection of prototype of Basic Life Support

(BLS) Ambulance & Advance Life Support (ALS) Ambulance

was held at M/s Ahmed Medix, Lahore dated 30.03.2016.

During the inspection of BLS following distinctions were

found;

Out Side Design

i. Name & Logo (design) of institutions to be printed on the

Ambulances should be provided to the M/S Toyota Ravi

motors and they will be pasted on the pre-decided place.

Four Person Seat

i. M/S Toyota Ravi motors was directed to use genuine

seats of Van instead of own made seats (2x2seat) and

make it convenient as per requirement of signed contract.

One Person Crew Seat

i. M/S Toyota Ravi motors was directed to use genuine crew

seats of Van instead of own made seat and make it

convenient as per requirement of signed contract.

Out Side Design

i. Name & Logo (design) of institutions to be printed on the

Ambulances has been requested to the DGHS (Punjab).

(Annexure-A)

Four Person Seat

i. No change made by the firm. Firm is again directed to use

genuine seats of Van instead of its own made seats

(2x2seat) and make it convenient as per requirement of

signed contract. The same was required in the advertised

specifications and further specifically mentioned in the

technical evaluation report as well, which was uploaded on

the health department website. Firm did not objected on

this point at that time. (Annexure-B)

genuine crew seats of Van instead of its own made seat.

The same was required in the advertised specifications

and further specifically mentioned in the technical

evaluation report as well, which was uploaded on the

health department website. Firm did not objected on this

point at that time.

Main Cabinet

i. Both of the cylinders (48CFT) should be made of USA

Origin and Tested from BOC. Their Certificates are

mandatory. The existing supplied cylinders brand and

origin are not available / mentioned.

ii. M/S Toyota Ravi motors was directed that Cylinder

brackets should be of S.S material instead of any other

material.

iii. Bull nose valve of the cylinder need to be replaced and

should be according to the issued order.

Lights

i. Strobe lights should of red color and at least of power

rating 35watt each, as per requirement of contract. The

existing are in two colors blue and red.

ii. The wiring sizes and standards were not in accordance

with the issued purchase order.

Utility Panel/Console

i. Oxygen flow meter is made of CBMCARE, Italy instead of

Hersill, Spain. M/S Toyota Ravi motors was directed to

make it according to signed contract.

ii. Oxygen Pressure Gauge, Low Oxygen Alarm system and

Main Cabinet

i. No change found. Firm was again unable to provide the

evidence that the supplied Cylinder is of USA Origin. Firm

did not provide any credible evidence which indicates the

brand and origin of cylinder. Firm showed BOC, Pakistan

testing certificate of cylinders which was last year dated

and irrelevant to the cylinders presented before the

committee. (Annexure-C)

ii. Oxygen Cylinder & their piping presented before the

inspection committee were not according to the B.S

Standard color coding. Firm advised to make it as per B.S

Standards for the clear identification of oxygen pipe.

iii. The committee checked and advised the firm to use a non

magnetic stainless steel instead of the existing one, which

will ensure the resistance of rust.

iv. The firm replaced the bull nose valve of cylinder which is

acceptable. The replaced was of O-Two Canada brand.

v. Firm directed to use two switches for two sockets in main

cabinet instead of one switch of two sockets.

Lights

i. Firm did not change the same. Firm was again directed to

make it a single red color strobe light. Moreover, No

documentary evidence provided by the firm and no

physical check /test were performed by the company to

check the power rating of 35 watt of strobe light.

ii. No change made by the company in wiring. The wiring

sizes and standards were not in accordance with the

issued purchase order.

Utility Panel/Console

i. The firm did not replaced the flow meter as per issued

purchase order of Hersil, Spain. Firm was again directed to

make it according to signed contract.

ii. No credible evidence provided by the firm that the
