Module Objectives:

1. The student will analyze epidemiological data to support and define the development of Program

design and implementation. (CO 1)

2. The student will identify programs and sources influential and supporting of proposed program

implementation. (CO 3)

3. Synthesize evidence-based and evidence-informed data to guide population health interventions

and programs. (CO 4)

• Cupp Curley Ch 10 & 11

• Gordis None this week. Your next Epi Quiz 3 is Wed, April 14.

• Community Health Needs Assessment (CHNA or CHA) you previously located in Module 4.

You have been exploration different needs and possible interventions for your population of interest.

1. As an APRN, what information about demographics of your Population of Interest (POI) can be

found in the (report form both): POI- African Americans females

A. U.S. Census Bureau about the demographics of the county and/or

B. County Health Ranking about health outcomes and social determinants of health (SDH) for

the county (www.countyhealthranking.org)

2. From the CHNA you located in Module 4, what are three of the priority health areas for the

county area? Is your POI or intervention in those noted priority health areas?

3. In your text, review and explore the Sources of Community-Level Indicators (p 252-253).

A. Offer at least 1 new source of further data. Supply the web address.

B. Report the type of data is available from this source.

C. Note: If this list is not relevant to your POI or topic of intervention, you may offer a new

source outside of this list. Supply the web address.

4. Problem-Priority Criteria (p 272-280)-Whether the priorities are focused on the coalition’s actions

for the communities or institutions/health-care facilities in the communities, there is criteria for

the priority setting of the problem. Based on the data you have located to this point, choose

(identify), report, and discuss 1 of 3 of the approaches relevant to your proposed need or

program (p. 275-277):

A. Problem Priority Criteria

B. “Skilled” Planner Approach

C. Hanlon Method and PEARL

5. Recall the community known to you. Consider your POI (African American Females)and

intervention( Improving management of prediabetes by increasing screening and treatment),

which community leaders, healthcare providers, organizations, and possible funders would you

initially contact to introduce this idea? (This could be the group that collectively determines the

Logic Model, tiered steps in a long-time project, have ideas for other members.)
