Change Theory Application
The change I made to our nursing practice that has a good impact to our workplace:
Comprehensive measurement of hourly intake and output for our patients. The purpose is
to avoid the risk of fluid volume overload. The steps I took was first: inform the nurses
about the significance of measuring the intake and output of patients.(please help me
explain the significance) Second is create an audit tool that can easily be used and the
results can easily calculated by the auditing team, then do a preliminary audit of the
documentations of the nurses to their assigned patients. Third is present the data to the
nurses and the impact of their non-compliance to the comprehensive documentation. Site
example of a situation of a patient with fluid volume overload of a patient with
congestive heart failure (_what does fluid volume overload do to a patient with
congestive heart failure? Please elaborate). Then announce that re audits will be done,
and nurses are expected to properly document Comprehensive hourly intake and output.
And a written discussion will be made to those noncompliant nurses. Explain to the
nurses that this change will be uncomfortable at the beginning but when they get used to
it, it will just be a part of their routine documentation, and will help the patient to be
provided with quality care from our organization by avoiding faults of fluid volume
overload.
The result of these effort to provide a good quality care to our patient by implementing a
well thought approach to change was a success. Now it is part of the nurse’s routine
documentation to assess hourly intake and output, it only takes them a few minutes to
document. This documentation has provided the team to look back on how many fluids
has the patient received, if is it enough or deficit. How many fluid/blood the patient lost
and apply the right interventions and treatment to the patient. This was a great way to
level up our patient care.
The change theory I used and apply to the practice in my workplace: (explaining root cause

Shows evidence of problem-solving skills by presenting comprehensive ideas and
information related to analysis of the root cause analysis.
Explain the change process used (steps) for meeting the outcome that needs to be
changed or improved. Must demonstrate evidence-based practice. Use evidence to
support your interventions.
The three Articles I used:
1.Article one - will explain the change theory that I used for the change in my workplace.
2. Article about Fluid volume overload on Congestive heart failure, maybe a little
pathophysiology.
3. Any Change Theory article(heathcare leadership/administration) that will apply to my story.
