Patient HM has a history of atrial fibrillation and a transient ischemic attack (TIA). The
patient has been diagnosed with type 2 diabetes, hypertension, hyperlipidemia and ischemic heart
disease. Drugs currently prescribed include the following:
Warfarin 5 mg daily MWF and 2.5 mg daily T, TH, Sat, Sun
Aspirin 81 mg daily
Metformin 1000 mg po bid
Glyburide 10 mg bid
Atenolol 100 mg po daily
Motrin 200 mg 13 tablets every 6 hours as needed for pain
Risk factors for transient ischemic attack (TIA) include a family history of stroke or TIA,
age above 55 years or older, higher risk of TIA in males than females, high blood pressure,
diabetes mellitus, and tobacco smoking. Genetics, race, and imbalance in lipid profile are other
risk factors for TIA (McCance , Huether, Brashers, & Rote, 2014). The risk of a stroke in atrial
fibrillation depends on your age (you have a higher risk as you age) and on whether you have
high blood pressure, diabetes, a history of heart failure or previous stroke, and other
factors (McCance et al., 2014).

A controllable risk factor for this patient would be controlling their weight by living a
heart-healthy lifestyle. You can help prevent a transient ischemic attack (TIA) or stroke if you
control risk factors and treat other medical conditions that can lead to a stroke. You can help
keep your heart and blood vessels healthy by taking steps toward a healthier lifestyle. These
healthy habits include not smoking, eating right, exercising regularly, staying at a healthy
weight, and getting the screening tests needed regularly (McCance et al., 2014).
