I. Specific Aims, hypotheses/objectives - Luann
Describe the (notional) community where this project will occur: demographics, socioeconomic
conditions, findings of previous community health assessments; (1 page).
Things to think about: we have to narrow a “specific” population
- Are we an organization? What type? We could be a school as we talked about
So, is the school applying for the grant? If so..
We have schools that teach 5-14 years - THIS COULD BE THE TARGET POPULATION
Goal: EXAMPLE - What do you think?
This project will target economically disadvantaged school-aged children from
“kindergarten to junior high?”. It will ensure that students have access to physical and
nutritional programs that extends into lunch period, after school and summer
opportunities. The lack of school policies and environmental supports for physical
activity and healthy eating contribute to this ongoing epidemic of obesity among school-
aged children; especially those with lower socioeconomic status. The goal is to teach
healthy behaviors and social skills to increase physical activity and nutritional eating

and our community at the (school name) is no exception. Racial/ethnic minority among
African Americans and Hispanic school-age children are most at risk for unhealthy
weight gain. Obesity is a risk factor for health outcomes later in life and this program will
provide dietary and physical activity behaviors to improve these outcomes in our
community. Currently there are _____% of our students that are overweight or at risk for
becoming overweight according to ________. Student fitness scores reveal that only
_________% of our students demonstrate performance in the “healthy fitness zone” as
indicated by the latest state physical fitness testing. Quality daily physical education has
been cited as schools’ best approach to prevention and intervention in the childhood
obesity crisis.
II. Public Health Significance - Obesity ( > heart disease, type II diabetes etc.)
(Population) Socioeconomic inequities (lower educational achievement, occupation and
income)
Briefly identify the public health concern to be addressed. What is the at-risk population for the
negative outcome? . What is the prevalence, incidence, and severity of the problem? What are
the short term and long term outcomes of concern?; (approx. 2 pages).
