Community Health Assessment
Community health assessment provides people living in the community and law makers
with information and data on the community's health needs and concerns and the examination of
existing resources to address the identified health needs and concerns. The community
assessment also helps identify the strengths and weaknesses of the community’s ability to make
change (Clark & Paraska, 2014). Individual health assessments might be different from the
community health assessment because they may not reflect the wider health needs of the
community. The nursing process should be included in all health assessments regardless of
whether it’s the patient, their family, or a community.
The first step of a health assessment is assessing the needs and risks to determine the
overall health of the community; cultivate a plan; involve the community; identify health
indicators; collect and analyze data; identify health priorities; and evaluate and report results.
Maurer & Smith (2013) discussed “determining risk status of an individual or community is
based on an analysis of sociodemographic characteristics such as age, sex, ethnicity, cultural
practices, and environmental hazards, and can be used to determine assets or gaps in an ability to
meet community needsâ€.
Community statistics or demographics is information about the population. The United
States Census Bureau (n.d) website is a valid and reliable resource to obtain the community
statistics or demographic information. Community statistics or demographics information that
would be helpful to gather besides the total population include sex, gender, racial and ethnic
background, education, income, employment status, marital status, family size, education.
