Rectal Examination
Steps
A rectal examination is a quick and painless process wherein a nurse or doctor uses their
finger to check for issues inside a patient's bottom (Stephan et al., 2018). The first step in the
examination is asking the patient to lie or stand in a suitable position. There is the standing
position wherein the patient stands with toes pointed inwards and then leans over a table. Also,
there is the knee to chest position in which the patient lies on a table while bending his or her
knees forward to the chest level. Further, there is the lateral decubitus position, also known as
sims, which is usually the most preferred wherein the patient lies on his or her left or right side
with one hip straight and the other bent depending on the side on which he/she is lying in. once
the patient is in a proper position, lubricate the index finger and gently insert the finger after
preparing the patient of such invasion. The external sphincter tone is then appreciated, with the
patient being asked to bear down. They should also be able to feel for sphincter tightening in the
process. Next, palpate the prostate gland and feel for tenderness, nodules, and proximate gland
approximate size (Stephan et al., 2018). The rectal wall should then be palpated both clockwise
and anticlockwise to feel tenderness, masses, and nodules. The finger should then be removed
and checked for any fecal material, then subjected to a guaiac kit. This marks the end of the
rectal examination.
Rationale
This examination is essential in the prescreening of nodules or enlargements in the
prostate and rectal areas. The insertion also allows for identifying any cause of rectal bleeding,
which is why the finger has to be gloved and lubricated to prevent it from causing bleeding and
leading to infection (Stephan et al., 2018). Further, it is critical to understand how the palpate
