AMOEBIASIS PRESENTED BY AGBEDE DAMILOLA JOHNSON OUTLINE • • • • • Introduction Pathogenesis Clinical Features Investigations Management INTRODUCTION • Amoebiasis is caused by Entamoeba histolytica; it is common throughout the tropics. • Infection can give rise to amoebic dysentery or extra-intestinal amoebiasis, e.g amoebic liver abscess. PATHOGENESIS • Amoebic dysentery/amoebiasis • Cysts of E. histolytica are ingested in water or uncooked food contaminated by human faeces. • The parasite invades the mucous membrane of the large bowel, producing ulceration • Incubation period of amoebiasis ranges from 2 wks to many years, followed by chronic course with grumbling abdominal pains(often mimicking appendicitis) PATHOGENESIS CONTD • Amoebic Liver abscess • This occurs when trophozites enter the liver via the portal vein. • In the liver, usually the right lobe, they multiply and rapidly destroy the parenchyma, forming an amoebic abscess. CLINICAL FEATURES • Intestinal amoebiasis: • Abdominal pain(mimicking appendicitis) • Diarrhea alternating with constipation • Passage of blood and mucous • Amoebic liver abscess • Hepatomegaly • Tender liver • Cough • Pain in the right shoulder A Large liver may rupture through the diaphragm into the lung, from where its contents may be coughed up INVESTIGATIONS • Microscopy: fresh stool sample may reveal motile trophozoites. • Sigmoidoscopy • Immunoflourescence: antibodies detecta ...
