Summary Statement
The Case Manager provides a comprehensive case management for patients that are
chronically ill or are disabled adults, adolescents, or children and their families. The key goal
is to improve the quality of the patient’s life, maintain peak ability to constructively function
within the community. However, it is imperative to note that the Case Manager works under
indirect supervision from the administration.
Duties and Responsibilities
1. Enable and facilitate the client’s access to community resources. Some of these
resources include housing, clothing, food, recreational activities, vocational
opportunities, and school programs. In this same aspect, the manager helps the client
to make the required social support networks.
2. The manager is responsible for coordinating and monitoring services. He or she
comprehensively tracks client activities based on the case plan.
