Grouping populations helps a lot in improving the care of patients. Some methods that

can be used in grouping patient populations are disease, age, social factors, demographic factors,

utilized when segmenting the patients include age and disease with their risk stratification.

Grouping populations into different categories helps a lot in addressing the high cost that is

incurred when managing various chronic illnesses.

The Healthcare system should also focus a lot on utilizing evidence-based screening and

prevention strategies because prevention strategies in the healthcare system are of paramount

importance. Preventing illness amongst high-risk patients is crucial because it helps in reducing

healthcare costs. Healthcare providers should always utilize evidence-based preventive strategies

amongst healthy individuals and even low-risk populations in their entire lifespan. Doing this

will help ensure that certain chronic illnesses are avoided by all means possible.
