The Bloom Program is a community- and pharmacy-led program created to care for
patients of addiction. The article evaluates the program seeking insight on caring for the patient

pharmacists assists in the medication management of patients, most notably those with chronic
diseases. A community approach proved more effective in reaching the clients due to improved
trust between the pharmacists and patients. The community opened up more about their mental
