M 1 Rheumatic Heart Disease   Inflammatory process that may affect the myocardium, pericardium and or endocardium Usually results in distortion and scarring of the valves 2  M:F ≈ 1:1, but poorer prognosis in females  Median age – 10yrs, 20% of cases occur in adults  Diagnosis of RHD is made after confirming antecedent RFthe modified Jones criteria  Evidence of previous GABHS pharyngitis: 1. 2. 3. Positive throat culture or rapid streptococcal antigen test result Elevated or rising streptococcal antibody titer History of previous RF or RHD 3 Cardiac Manifestations of Acute RF  Pancarditis - most serious complication of RF (50%) detected by a new murmur & tachycardia out of proportion to fever  Advanced cases: dyspnea, mild-to-moderate chest discomfort, pleuritic chest pain, edema, cough or orthopnea  Echo-Doppler evidence of mitral insufficiency in association with AI, may be sufficient for a diagnosis of carditis (even in the absence of accompanying auscultatory findings)  Other cardiac manifestations include CHF & pericarditis 4 Rheumatic Heart Disease, cont.  Subjective symptoms    Prior history of rheumatic fever General malaise Pain – may or may not be present  Objective symptoms     Temperature Murmurs Dyspnea polyarthritis 5 ..
