Week 6 Assignment: EBPChange Processform
ACE Star Model of Knowledge Transformation
Name: _____________________
Star Point 1: Discovery (Identify topic and practice issue)
Identify the topic and the nursing practice issue related to this topic. (This MUST involve a
nursing practice issue.)
The aspect selected involves safety in a healthcare environment. Besides, the nursing
practice issue to be addressed is hospital-acquired infection (HAIs). In this case, HAIs refer to
the diseases that healthcare professionals and patients contract while within the healthcare
setting. There are several aspects that contribute to contracting these infections, where some of
them include failure to use personal protective equipment appropriately, keeping close distance
with the infected individuals, lack of washing hands appropriately, and unclean working
conditions.
Briefly describe your rationale for your topic selection. Include the scope of the
issue/problem.
The main reason for the selection is that HAIs have significantly increased among
healthcare providers and patients. However, the healthcare providers, especially the nurses,
believe that they cannot contract illnesses because they are the ones who treat diseases. However,
they are not aware that diseases do not differentiate between patients and healthcare providers.
They usually occur to anyone who is exposed to them. Therefore, I have selected the topic to
create awareness among healthcare professionals that they have equal chances of contracting
infections just like other people, regardless of their role.
Star Point 2: Summary (Evidence to support the need for a change)
Describe the practice problem in your own words and formulate your PICOT question.
Individuals found within the mentioned environment are among the people who are
susceptible to various infections. This is because patients visit the area with several infections
where some are identified while others are not. Healthcare professionals have equal chances of
contracting illnesses as patients, and therefore they should take great precautions since they can
serve as the medium for the disease. In this case, they can pass the infections from patients to
patients or other healthcare professionals. Based on the issue under concern, a PICOT question

that can be developed how: Can healthcare professionals and patients (P) be prevented from
HAIs through education (I) during their interaction (T) in healthcare facilities setting to avoid the
increased spread of diseases (O) compared to the one experienced outside the healthcare
environment?
List the systematic review chosen from the CCN Library databases. Type the complete
APA reference for the systematic review selected.
The article selected involves the proportion of HAIs that can be prevented.

List and briefly describe other sources used for data and information. List any other
optional scholarly source used as a supplement to the systematic review in APA format.
During the searching process regarding prevention of healthcare professionals from
HAIs, I found an article involving reducing HAIs to get hospitals and the healthcare system back
to zero. This scholarly source was closely related to my PICOT question since it applied different
ways of reducing HAIs, including education.
The other scholarly source that I obtained related to my PICOT question involved
implementing a CAUTI prevention Algorithm to reduce HAIs. During the adoption of this
prevention measure, several strategies were applied, where education was among them. This
implies that there is enough evidence that HAIs can be prevented through education and other
strategies.

Briefly summarize the main findings (in your own words) from the systematic review and
the strength of the evidence.
