Running Head: COMMUNITY NURSING 1 Community Nursing Institutional affiliation Date COMMUNITY NURSING Educational preparations and skills recommended for case managers Case management, as described in chapter 9, is comprised of several programs in acute hospital as well as community settings. Case management mostly influences the health of a community, which encompasses patient-centered medical homes, ambulatory services, occupational health, among other health services required by the community to better their services. Care management is usually made up of programs that are connected to different systems. The systems include and not limited to information, science, methods, and incentives that are provided to enhance the effectiveness of medical health provision among individuals to ensure that clients and support systems can collaborate and participate together to improve health, social, among other health conditions of people. According to recent studies and innovations, case management has been identified as a concept which is emerging as an evidence-based, patient-centered, and clinical care focused. In this perspective, healthcare practitioners should have the required knowledge and have the responsibility to administer possible healthcare to a patient (Nies, Mary A., McEwen, Melanie, 2014). His eyes became red today, so he decided to consult to get evaluated. On his physical assessment you found a yellowish discharge and bilateral conjunctival erythema. His throat and lungs are normal, his left ear canal is within normal limits, but the tympanic membrane is opaque, bulging and red.1.- Based on the clinical manifestations presented on the case above, which would be your eyes diagnosis for C.J. Please name why you get to this diagnosis and document your rational.2.- With no further information would you be able to name the probable etiology of the eye affection presented? Viral, bacterial, allergic, gonococcal, trachoma.
