Provide a brief context for the setting in which the event took place. Keep this short and general. The sentinel event that the root-cause analysis (RCA) will explore is that of a patient who was identified to have a retained gauze some days post negative pressure wound therapy (NPWT) of a wound that had become infected and primary closure was taking too long. The event took place in a county referral hospital and was recognized when the patient was brought back for follow-up some days after discharge. Event Description Describe the issue or sentinel event for which the RCA is being conducted. Provide a clear and concise description of the problem that instigated the RCA. Your description should include information such as: ● What happened? A 66-year old male was admitted a second time to the referral hospital with a wound that had become infected and had delayed primary closure after excision of a large seroma in the abdomen. It was agreed that patient be put on NPWT on the same day of admission, which was replaced with a wet to dry dressing just before discharge three days later. Finally, what impact did your project have on your target population? Please submit a recorded presentation of between 7 and 9 minutes that is a reflection of the project based on a full representation of all three parts above. You may video yourself discussing the project or you may create a PowerPoint presentation and record your voice to the presentation. Use a recording platform of your choice and either upload as an mp4 or share the link directly to the video in the dropbox. ***Please do not record as voice-over PowerPoint because this cannot be saved in mp4 format or a link.*** If you submit your assignment as a powerpoint with voice over recording you will not receive credit for your assignment (or partial credit as you did not meet the full requirements of the assignment.The presentation should include at least two scholarly sources other than provided materials.
