determine the prioritization process upon community health assessment analysis.

Community Health Assessment Analysis

Community health assessment is a critical process in promoting healthcare in the

community. When conducting a community assessment, it is crucial to set realistic goals and

establish the most effective way of engaging the participants (Clark, 2015). For instance, I

chose friends from diverse backgrounds, so I must be culturally sensitive when addressing the

issue of cardiovascular health. The initial preparation is critical before implementing

prioritization techniques because it helps select the most appropriate health issue to be

addressed. Step five of the Community Health Assessment Toolkit focuses on prioritizing

community health issues (AHA Community Health Improvement, n.d.). Step five of the toolkit

is to help community stakeholders and hospitals to address the identified health need from data

collected using the implementation strategies. The toolkit offers various prioritization criteria;

selecting the most applicable criteria helps a nurse to analyze areas that may negatively affect

health outcomes and ensure critical health issues facing the community are addressed first.

My criteria will be to identify the severity of the problem; this will involve identifying

the population in the community diagnosed with cardiovascular disease. Then I will identify

the knowledge base of their cardiovascular health as well as their views about the diseases.

Prioritization of the need will follow and formulate a plan of addressing the need gap. I will

implement change based on the health need to promote and improve community health. In

addition, ensuring community views are adequately included in the implemented change helps

to increase community compliance. The other criteria I will consider is if there are hospital and

community resources for addressing the issue. Thereafter, I will conduct an analysis to identify

if there are programs or plans implemented by the federal or state government to address

cardiovascular-related diseases. This strategy aligns with one of the key components of identity

criteria for prioritization which focuses on identifying existing interventions initiated to

address the issue (AHA Community Health Improvement, n.d.).
