R.T. is a 35-year-old female patient who was seen in the clinic because she has been
trying to get pregnant for about 7 months, however, she has not been successful. Only positive
gynecologic history for R.T. has been three episodes of pelvic inflammatory disease in the last
five years.
· Discuss A Diagnostic Plan For R.T. and the Rationale.
Investigations
a) Urinalysis will be considered to check for urinary tract infection (UTI) (Mitchell &
Prabhu, 2013).
b) A full hemogram will be needed to rule out disseminated blood infection.
c) C-reactive protein as an inflammatory marker
d) The high vaginal swab will be necessary to check for any infection along the urinary tract
(Mitchell & Prabhu, 2013).
e) It is essential to check for Venereal Disease Research Laboratory (VDRL) to check for
syphilis.
f) Imaging, including transvaginal ultrasound, would check for pelvic inflammatory disease,
any endometrial abnormality, or any pelvic masses.
g) Cervical excitation tests to check out for cervical tenderness.
Discuss Your Presumptive Diagnoses and ICD 10 Number.
In this case, the presumptive according to ICD 10 is N7.9- Female pelvic inflammatory
disease, unspecified (WHO, 2010). This is the presumptive diagnosis before workups are done.
