The increased medical costs in United Stated have deprived citizens of seeking
appropriate care. Despite continued efforts to improve healthcare access and quality of care, the
healthcare system's fragmentation poses a barrier for people suffering from chronic disease to
receive adequate care, mainly the minority groups and common among mental health patients.
Besides, the stigma around these populations often a barrier to mental health patients seeking
treatment. Without adequate intervention for meeting the population's health needs, chronic
diseases' prevalence will continue to rise and continue to create a financial burden on the nation.
Evidence has shown meeting people in their community where they live, and work encourages
participation in their disease management. Exploring new innovative models will reduce the
health disparities seen among the vulnerable populations and people suffering from mental
diseases.

Mobile Health Clinic (MHC) is an innovative healthcare delivery model that
provides care to residents in the community. The study showed that mental health patients are
more compliant with their disease management when receiving care in their comfort zone.
Besides,
bringing care into the community reduces travel times for the patients and their families, reduces
stigmatization, and enhances care continuity (Yu et al. 2017). This paper aims at establishing the
Innovation of mobile health clinic-bring care to the community to address the health disparities
among the mental health populations and underserved populations. The goal is to provide quality
care, decrease readmission rates, and provide continuity of care in these populations.

Role of Innovative Nurse Leader
The increased workloads and continuous changes in healthcare systems have prompted
nurses to frequently evaluate how patients' needs are met, improve patient care and outcome of
care, and indirectly reduce medical costs and improve employees' job satisfaction. Snow (2019)
addressed nurse innovators as agents of change in leading processes and strategy and influence
technology that adds value to patients' health outcomes, cost-effectiveness, and improved
satisfaction for both staff and patients. Nurse innovators ally with other leaders/stakeholders to
move innovative ideas forward. Nurse innovators advocate for legislation that provides equal
care across the board regardless of an individual's economic status. Adept in leadership with
solid communication skills; this helps present innovative ideas to the lawmakers' stakeholders.
Innovators demonstrate a role model attitude the employees can emulate.
Changes require persistence and strong leaders with the same mindset; however, many
healthcare systems change gear towards providing quality care to the consumers. This
innovative is moving care to the community aiming toward mental health patients in the D.C.
metropolitan.
