Fall Intervention Nursing Care Action Plan
Fall prevention by involving older residents and their families.
Involving patients and their families in conversations about fall risk and mitigation
improves collaborative communication. One cornerstone of my fall intervention nursing care
action plan is enabling my patients and their families to participate as partners in the fall
intervention strategy. Fall-related accidents can be successfully reduced if patients are motivated
to participate in personalized treatments (Vonnes & Wolf, 2017).
Using strategies that have a good chance of succeeding, based on the data.
The study of health practitioners' practical experiences and perspectives has been
highlighted as a way to improve knowledge translation (Van Rhyn & Barwick, 2019). As a health
professional, I am a major player in the practice of incorporating fall prevention data into
therapeutic assessments and care of older persons at risk of fall.
Leveraging existing community resources to prevent falls.
Community health improvement resources are one of the many advantages that
professional practitioners may use to increase the determinants of health in the areas where they
operate (Kaplan & Gourevitch, 2020). In my case, there is a vast network of institutional and
community resources to assist seniors and people with disabilities. In a state, competing healthcare
institutions have a history of working together on major health and safety problems such as elderly
patient fall prevention. I will make an effort to make sure that I maximize the use of available
resources in the community to strengthen my fall intervention nursing care action plan.

I will implement the following fall protection measures: • Realigning the patient to their settings, notably the placement of the restroom and the usage of a call light. • Unless performing a treatment on a patient, keeping the bed in the lowest setting while in use. • Maintaining the top two side rails • Beds, prosthetics, and walker are all secured with latches. • Keeping the flooring clean - no debris and obstacles • Putting a call light within easy reach of the patient. • Answering the phone as soon as possible. • Encouraging the patient to seek help when possible. • Ensuring enough lighting, particularly at nightfall. • Having non-skid footwear that fits properly.
