Student Assessment Data Review
During the 2020-2021 school year, Valentina was an eight-year-old second-grader. She
had difficulty recalling text details, counting, blending, and segmenting syllables. On the
Istation’s Indicators of Progress (ISIP) assessment her overall reading score was 208 which is a
level 1 out of five levels. Valentina consistently scored a level 1 all school year and did not show
adequate growth. She also took the Exact Path diagnostic assessment in reading and scored in the

national percentile with a numeric score of 821. Although she was a second-grade student
her exact path for learning included mostly Kindergarten reading foundational skills. The focus
for her RTI plan was on phonemic awareness, syllables, blending, and segmenting syllables.
