One of the major issues that healthcare industries have to face are the errors when the release of

information is necessary. This includes patient personal data, treatment plans, payment details

and other restricted information. The cost to remedy a breach when we are talking about ROI is

massive and because of that, solutions to lower the frequency of errors should be found and get

implemented in the system.

The first things this article refers to are the policy and the procedures imposed by the health

information management. The first essential idea is that these ROI policies and procedures

should be adaptive in order to be able to adapt them to any new implementations. Secondly , the

training represents another key factor that can reduce the number of breaches. Everyone who

wants access to a patient file should first be validated through and ID card and the information

that is presented to that person should be limited to only the requested ones and no other details

should be included. The personnel who is working at the front desk should always make sure that

they use enough identification factors in order to avoid any mistakes. These factors can be

represented by the patient's full name, the social security number and the date he or she was

born.When referring to workflow the two improvements consists in creating a streamlined

process and after that testing that process and measure the chances of breaches. All these testing

should be done at least once every year. The article provided us with a few examples regarding

disclosures mistakes. One of them is represented by the situation when the wrong chart is sent to

the person that requested it. Another one is a result of the confusion that can be caused by two

charts of different patients which have the same record number. The last one, and probably the

one that is easiest to understand is when we have multiple family members since their name can

create errors.
