Realizing that you will need to add Querying to your process list, clearly identify the intent
of queries included in a procedure format and provide examples of why a provider might
receive a query.
The intent of queries
- To provide clarity when diagnoses has been listed without a statement of cause or without
cause. When procedures are not clearly documentation.
- To understand illegible data that has been written by hand and cannot be accessed for
coding
- To seek correctness when there is a need for more specific documentation
- For consistency when information documented is conflicting or has not been well
explained.
- To seek completeness when there are entries to patient records that do not correlate to
diagnostic tests.
Reasons why a provider may receive a query
- When the records show indicators of a diagnosis but the diagnosis has not been
documented
- When there is clinical evidence for a higher degree of severity and specificity in the
diagnosis that has been presented.
- When there is a cause and effect between two conditions, hence clarity needs to be sought
out
