Expected outcome
The desired outcome is for the patient is to have an improved thought processing ability
exhibited by good judgment and orientation to time, place, person and circumstance within
twenty-four hours. The patient will also identify and control the desire to move about throughout
the hospital stay.
Nursing interventions and rationale
Inform the patient about the surrounding every two hours and allow having familiar
objects in the surrounding to achieve maximum orientation to reality. Also, the patient should be
allowed to wander in a controlled environment and within acceptable limitations to decrease
hostility and agitation and at the same time maintaining security with difficult to control
behaviors (Butcher, 2018). In addition, communicate with warmth with the patient and offer
support in decision making to prevent feelings of loneliness and frustration.
Evaluation tool
The expected outcomes will be assessed by assigning points in percentage to the assessment
factor depending on the level of improvement. Factors will be assessed out of 100% with 1-44%
representing unmet goals, 45-55% fairly met goals and, 56-100% fully met goals.
