Workflow improvement is attained by examining every phase in practice and applying

the workflow, one has to comprehend the existing procedure that is being executed. Flowcharts
offer a simple means to get this understanding by planning every stage of the assignment from the
start to the completion (Cain & Haque, 2008). Thus, this task is purposed to form a flow chart of
activity, examine every step in practice, and propose modifications that will enable developments.
Explanation of the Flowchart
Figure 1 presents the recent workflow process selected for this task. The workflow process
presented in the flowchart is the decision of the hospital to discharge a patient. Whether the
discharge is at home, a rehabilitation center or a nursing home, the health, and safety of a patient
should be given proper consideration. Improvements in hospital discharge organization can
intensely recover the result for patients as they go to the next level of treatment. Patients, family
and healthcare providers all are important in maintaining the health of a patient after discharge
(Cain & Haque, 2008). Even though it is an important part of the entire care plan, there is an
