What is the difference between self-management support and patient education?
Self management support is the help, care and encouragement given to people who have
chronic conditions together with their families. This support is aimed at helping such people and
their families to understand their role in the management of their illnesses, making informed
decisions about care and engaging in healthy behaviors. On the other hand, patient education is a
process by which health practitioners convey information to patients as well as their caregivers
so as to help them improve their health status or change their health behaviors. It is provided by
any healthcare practitioner who has undertaken the necessary training education. Patient
education includes either or both general preventive education (health promotion) and disease
(condition) specific education. The main difference between self management support and
patient education is the group of people which is focused on. Self management support is
provided to people with chronic conditions while patient education is provided to people for
preventive reasons or to those with conditions.
What are the telehealth applications for self-management? Can you give at least two
examples?
Telehealth is a collection of means or methods for the enhancement of healthcare, health
education delivery, public health and support using telecommunications technologies. Examples
of telehealth applications include live video (synchronous) and store and forward
(asynchronous). The live video (synchronous) application is a two interaction between a patient
and a care giver or provider using the technology of audiovisual telecommunications. This
application is also referred to as real time and can be used for the services of consultation,
diagnosis and treatment. This application serves as a substitute or an alternative of in person
