1965
Enactment of Medicare and Medicaid Program
During the tenure of President Lyndon B Johnson, he signed into law
the Medicare and Medicaid programs since there were inadequacies in the
medical welfare care initiative which was placed under the public assistance.
There was the need of ensuring improvement in efficiency and quality of
medical care distribution in the United States which led to the establishment
of Medicare program which offered medical insurance to the elderly and
disabled individual in the society to receive hospital insurance and
supplementary medical insurance (Grabowski, 2017). Through the Medicare
and Medicaid programs of President Johnson, it improved the quality of
health care since millions of Americans health was protected leading to
saving of lives and improving the economic security of the country. The
programs also required specific rules to be ascertained by the American in
ensuring that they receive quality medical care.
1968
Healthcare Quality Improvement Act (HCQIA
The introduction of the Healthcare Quality Improvement Act (HCQIA)
of 1986 came after many malpractices suits occurred in the United States
health care industry during the 1970s and 1980s which prompted the
Congress to engage in enacting the HCQIA to ensure there is the quality,
efficient and reliable distribution of health care services. Before enactment of
the HCQIA, the healthcare industry lacked a physician, tracking system
which was not held medical practitioners accountable for the services that
they provide to their patients (Horner, 1990). Before the act, many physicians
engaged in incompetence and malicious activities which have adverse effects
on many patients leading to the poor-quality distribution of healthcare in the
industry. The act led to the establishment of peer review boards which were
mandated to sweep the industry. Through the peer-reviewed boards, they

Part B: Continuous Quality Improvement Reflection
engaged in reviewing the negligence of the physicians in the industry and
provided recommendations on the actions that would be taken against the
physician's misconducts which helped to improve the quality of healthcare
services offered by physicians in the industry.
2002
Quality Improvement Organizations
In 1983 there was the establishment of the Peer Review Organization
(PROs) which led to the development of Quality Improvement Organizations
(QIOs) which engaged in defining the numerous proactive ways that would be
integrated into the health care industry to improve the quality of patient care.
QIOs was a regulatory body established by the federal government with the
intention of improving the quality of care for Medicare recipients together
with safeguarding the integrity of the Medicare Trust Fund (MTF). The
integrity of MTF was ensured through ascertaining that the payments which
are made by Medicare are only utilized for the stipulated purpose (Snyder &
Anderson, 2005). The QIOs towards ensuring there is an improvement in the
quality distribution of medical care engaged in addressing the Medicare
which recipients' complaints and overseeing the Emergency Medical
Treatment and Labor Act violation. QIOs also engaged in advancing
programs of the federal government which led to the improvement of the
quality of health provided in the community. The QIOs have also engaged in
collaborating with centers of Medicare and Medicaid towards improving the
quality of health care.
2004
The Affordable Health Care Act
The implementation of ACA also known as Obamacare led to
improvement in the quality distribution of health care since it made health
care to be accessible to all the citizens at affordable rates. The ACA also
improved the access of medical care apart from improving the quality of care.
ACA improved the quality of care in the industry since there were fewer
readmission rates and fewer infections which were caused within the hospital
walls which led to greater clinical outcomes, safety, efficiency, and patient
satisfaction. Many individuals through ACA the elderly individuals, children,
and the poor were able to access medical care at affordable rates and the
physicians were able to provide them with reliable and quality care thus, the
quality of care in the country increased (Koh & Sebelius, 2010). Previously
many individuals in the country were not able to access medical services due
to the high cost of medication; therefore, their quality of life was
deteriorating.
