We would like to thank Almighty God for guiding us and making this case

study a success. This case presentation about Congestive Heart Failure was made

possible through the effort of our team. The topic was very relevant since this has been a

killer disease common in most people.

We the group 3 of PN2-1 would like to express our heartfelt gratitude and

thanks to the people who have given their support in making this manuscript a wonderful

experience and to those people who became instruments in making this possible.

We would like to thank patient, for allowing us to have her as our subject for our case

study which she had been very cooperative and open during our interview.

We would like to thank the staff nurses of Isaac T. Robillo Memorial Hospital for

their kindness and patience in assisting us in every exposure that we have.

To our Clinical Instructor, for their advices, the knowledge they imparted to us,

and the time they taught us to be more responsible in every aspect that we do. Thank you
