Class I: Sodium Channel Blockers
Class II: Beta Blockers
Class III: Drugs that Prolong Repolarization
Class IV: Calcium Channel Blockers
Class V: Other agents
Most of the drugs used to treat dysrhythmias can also cause new
dysrhythmias or worsen existing ones.
Class I
IA: Slows Conduction & delays repolarization (blocks k+
channels)
Quinidine (can prolong QT)
Side effects:
Anticholinergic effects (pg. 555)
But can cause diarrhea (33% of pts due to
electrolyte shifts)
Tachycardia
Usually pretreated with dig, beta blockers, or
verapamil to counteract
Tinnitus
Vertigo
Interacts with digoxin
Class II
Nearly identical to Class IV: CCBs
In the SA node, these drugs reduce automaticity.
In the AV node, they slow conduction velocity.
In the atria and ventricles, they reduce contractility.
Prolongs PR interval on an EKG
Acebutolol
Propanolol
Esmolol
Sotalol (listed under Class III because it also blocks potassium channels)
