Patient-centered Health Interventions and Timeliness of Care
Patient Health Issues
The patient was diagnosed with chronic obstructive pulmonary disorder (COPD) a
lung disease where the airway is obstructed as a result of smoking. Common symptoms
associated with the condition include coughing mucus and wheezing. People diagnosed with
COPD are at a high risk of developing other conditions such as heart disease and lung cancer
as well as a variety of other conditions (Chabot et al., 2019). Therefore, it is essential that the
condition is appropriately managed. Currently, the patient struggles with applying the
requisite self-management skills. For instance, the patient has less information on the need to
adhere to various provided medication such as prednisolone, and ciprofloxacin. Also, the
patient has less education in regards to managing the huff coughs and also on how to deal
with the various deep breaths required in reoxygenating the lungs. Secondly, the patient has
less education on which interventions can be used to improve breathing based on their current
condition. Thirdly, the patient has not reduced the risk of further exacerbating the current
condition by failing to take the influenza vaccination which is known to increase symptoms
associated with COPD.
Intervention for Each Issue
Patient education significantly improves self-management practices among patients.
Patients can be educated on the importance of adhering to the required medical regimen
(Singh et al., 2019). The patient needs to understand the specific reasons for taking
prednisolone and its effects in reducing existing exacerbations, admission and also in
improving hypoxemia (Agrawal, Moghtader, Ayyala, Bandi, & Sharafkhaneh, 2019). The
patient also needs to understand why albuterol should be consumed after every 6 hours and
its overall effect on tolerating dyspnea. In addition, the patient should also be educated on
