Furthermore, the data must be veri?able and comprehensive. Imagine wondering what the cost for care is on a particular nursing unit only to ?nd some of the data missing or miscoded. Much of the data used to answer organizational and practice questions come from sources used for other purposes. For example, when a patient's weight is recorded in the medical record, the primary use for the data is in direct patient care. A secondary use might be to answer the clinical question "Do patients maintain their weight while on hyperalimentation?" When using data for secondary purposes, the individual's information is aggregated with a population of similar patients. By using quality data, the nurse leader is participating in knowledge discovery. "High-quality data requires a very clear understanding of the meaning, context, and intent of the data—unambiguous and, ideally, standardized computable definitions of data that can form the basis for future safe decision making" (Manos, 2012, para. 2). An evidence-based practice paper allows you to explore best practice and help improve client outcomes on a psychiatric unit.
