1.A title page with the title of your project,
your name, name of this course, and Kaplan University. Also a line with “date
submitted”

2.Titles for each section (example: U.S. Census
data on ____ county in the state of _____).

3.

Include questions 1-5 from Project 4.

4.The health assessment you designed in Project 6.

5.Add the information you have discovered to

6.Using the health concern of your choice (the
same one throughout this project) describe how you will gather data for your
health assessment (interview, focus groups, or whatever methods you feel are
appropriate).

7.Using the US Census information on your
community, complete the health assessment information. You may need to fill in
certain information that you may not have on hand (such as that you might
gather through interviews or focus groups).

8.After you have completed the health assessment
section, evaluate your information. (Consult Chapter 13 in your book and look
up a reliable example of a health promotion evaluation on the Internet.) Use
the link below for a guidepost for your evaluation. A good informational
website to help with evaluation is http://www.cdc.gov/healthyyouth/evaluation/resources.htm.

In Doc Sharing and below you will find worksheets to help you think about who
your health evaluation will affect. This was taken from
http://www.cdc.gov/getsmart/program-planner/Step1.pdf Complete and add this to
your assignment. This portion should include a plan of action.

9.A reference page with your sources of

sources.

How exactly you compile the material is up to
you but remember to give details and to submit material that is organized in
thought, has a professional look, in a logical order, and be thorough. Check
over your work before submitting to assure clarity, order, and accuracy.
Grammar, spelling, and punctuation will also be graded. As long as you include
the areas above, you may order the information as you wish as long as it
“flows” and is rational. Each component of the assignment should be organized
in thought, have a professional look, in a logical order, and be thorough.

This project will include your findings from
Units 4& 6. When completed, this project should be a picture of a health
assessment, how you have interpreted that data from Unit 4 from U.S. Census
data on your county, and evaluation plans.

Please note that there is no “one way” to
complete this project. Everyone’s efforts will and should look different.
However, each component of the assignment should be organized in thought, have
a professional look, and be thorough.

Your project should contain the following:

1.Questions 1-5 from Project Unit 4.

2.The health assessment you designed in Project
Unit 6.

Format your paper consistent with APA 6th

Community Health Interventions COMMUNITY HEALTH INTERVENTIONS 2 My county is Orange County FLORIDA. The county is one of the well-populated counties in Florida with a population of 1,225,267 as the total population. The whole state however has a population of 19,552,860. This is according to the census data of year 2013. Additionally, female persons occupy 50.7% of the whole population of the county with the rest being males. The population of senior citizens who are above the age of 65 years stood at 10.5% by the year 2013 while the children below 5 years and below took a percent of 6.3 of the whole population by the census of the same year (quickfacts.census.gov). Together with the one below the age of 18years, the percentage falls at 22.9% of the whole population. In Orange County FL, different groups of people are faced with different health risks. Each of the groups has its own health concerns and needs, which needs to be addressed by local health organization in order to ensure that they lives health lives and have access to better care and health facilities. One of the most vulnerable groups is the senior citizens, which is the section of the population at and above the age of 65 years. This section takes 10.5% of the whole population of the county. Most of the senior citizens are dependent and unemployed. They depend on the care from the family members as well as other care organizations. Among the health concerns for the people, the concerns include providing the elderly with the resources that addresses their needs that are unique for some seniors while others are more specific. These includes provision of seniors health care as well as the seniors care insurance for those not employed and have limited access to health care provisions such as Medicaid. The county also has concerns on providing free food to the seniors to ensure that the elders facing hunger crisis get the necessary and healthy foods. COMMUNITY HEALTH INTERVENTIONS 3 For the senior citizens, there are several specific programs in the county directed towards providing the necessary care for them. This also includes provisions for the health risk assessment as well as counseling and screening. Other care concerns include health promotions and dispensation of disease information throughout the county. To curb the issues related with the elderly health concerns, the Florida state, which is the location that orange county operates the Florida department of Elder Affairs, which coordinates the community nutrition services programs around the state. The department oversees the private agencies that run the services. Most of these agencies are non-profit organizations and senior living centers. There are also some established local area agencies offices that bring services closer to the population. Other includes senior volunteers and the charitable organizations across the county. Florida department for the elderly affairs ensures that the agencies provides a number of services to the elderly ensuring that anyone above the age of 65 and isolated gets the services delivered at their homes at their convenience. Other interventions includes offering preventable health services by specialists and medical professionals at clinics and senior centers as well as selected sites which is key to saving and reducing medical bills for the elderly. This is an innovative method in the whole state of Florida helping its elderly citizens have a healthier lifestyle. The department of the elder affairs also provides the aged and disabled adult waivers that helps the elderly to maintain independence and continue living in their own homes (Kandel and Adamec, 2009). It only accommodates the Medicaid eligible seniors and persons with disabilities and is vulnerable to health risks. It also helps in the provision of health and medical COMMUNITY HEALTH INTERVENTIONS 4 services to the less fortunate elderly people around the county. This includes operating the adult daycare, facilitating physical therapy as well as financial assistance for medical bills. Additionally, the department offers caregivers training services and support and free or low cost consumable medical supplies. Health risk management is a core activity for these organizations. Other intervention facilitated in the county concerning the elderly and the senior population of 65 years and older in Orange County and Florida as a whole includes facilitation of low cost legal services to the elderly. This is through the Florida senior legal service. These offers help the elderly to access their rights on serious civil matters related to their health conditions. These could be cases for damages causes through retrenchment, injuries, and disabilities among other health issues. With the provisions from the Serving Health Insurance Needs of Elders (SHINE) the department in the county is able to coordinate activities about the Medicare beneficiaries, family members as well as the care givers across the county in a most accessible manner.
