Project: Create a Software Application Module for Patient Data and a Wellness History
Synopsis: A physicians’ group has decided to update their technology to something more modern and
efficient. The segment of the task that requires your attention is in regards to the Patient Care System.
This system will need to collect information and store it into a database, as well as record wellness
evaluation data that is collected on each patient.
Description: A local physicians’ group, called LPG, is restructuring its internal technology processes. LPG
consists of approximately 25 physicians, lab and x-ray technicians, nurses and assistants, as well as front
office staff. In the community in which LPG operates, they serve approximately 3,000 patients, delivering
services such as emergency medical treatment, wellness checkups, disease management, as well as many
other medical offerings.
The company has chosen to create a new database system for patient record storage, as well as a new
user interface for keeping track of patients and their medical histories.
You work as an application software developer for an international corporation that has received the
contract to create the system, IBN. You are the primary developer and business interface between IBN
and LPG.
IBN’s specialists have already determined which software language and database that the application will
be created with. In addition, they have determined the first two modules that will be implemented: Basic
Patient Data and Profile, and Wellness Checkup Profile.
Basic Patient Data and Profile: This comprises the basic information collected regarding a patient. It will
encompass common information such as name, address, phone number, email address, primary care
physician, health insurance provider/group number/member number, and so on. It will also contain
profile information that will create a quick view of data collected from the wellness checkup, such as
average bpm, diastolic and systolic blood pressure, as well as most recent information such as weight,
height, blood pressure, current medications, and so on.
Wellness Checkup Profile: Every six months and/or when visiting due to illness, patients receive a
screening that collects basic information: Date and time of checkup, diastolic and systolic blood pressure,
heart rate, weight, height, current medicines, and so forth. This is a new record for each visit that will be
used to aggregate historical patient data. Each record is editable, but should not have its information
overwritten when the patient next visits, but instead receive a new record for each visitation date.
Deliverables:
1. You will inform LPG as to the database and software language chosen by IBN to create their new
application, in written form. You must detail the reasons that this particular database and
language were chosen, without being overly technical to the LPG Technology Committee that you
will be interfacing with.
a. A single Word document detailing these requirements is sufficient.
2. You must develop the database for the first phase of the project, providing the data structure for
Basic Patient Data and Profile, and the Wellness Checkup Profile. Tables should follow standard
entity-relationship industry practices.
a. Submit an entity-relationship diagram detailing data structure
b. Provide a backup file of the database with your implemented data structure
3. You must develop and deliver the operational front-end that allows for the entry of the data into
the first phase screens, in the language that you choose. It must be operational and connect to
the database that you created in Deliverable #2.
a. Provide all code and files in zip archive, required to run the project
4. You must provide the above documentation from Deliverable #1, with installation instructions for
implementing your First Phase deliverables for items #2 and #3.
a. Provide installation instructions to implement the system. This can be a single Word
document
