Instructions:
1) Create a form for any type of processing. Before coding, draw a layout of your form.
2) Be sure to include comments that include your name, period and date and indicates the various
fields set up for the form.
3) Post the form to http://ss1.ciwcertified.com/cgi-bin/process.pl to verify action.
4) The form should be broken into distinct categories.
5) The form should include the following parameters:
a. Input fields
b. Input field with max length and/or set value
c. Password field
d. Check boxes and radio buttons
e. Menu choice
f. Text Area
g. Field Sets with Legends
h. Submit and reset buttons
6) All fields have accessibility via short cut keys and tab index options.
7) Format the page using CSS style rules.
