Mrs G Client Centered Communication and Collaboration Case Questions
In this written assignment, you have the opportunity to share your thoughts about how to deliver client-centered culturally competent care and work collaboratively with others. The Case of Mrs. G.
Mrs. G. is a 75 year old Hispanic woman who has been relatively well all of her life. She had been married for 50 years and had five children. Her children are grown with families of their own. All but one of her children live in other states. Mrs. G.'s husband passed away last year, which was devastating for her. She had been very close to him and relied upon him for everything. He was "the life of the party" she always said and was a loving and caring man. Since his passing, Mrs. G. has continued to live in the house they shared for 35 years. In the last month, Mrs. G. has fallen twice sustaining injuries, though minimal. Her home health nurse comes weekly to check in on her. Mrs. G. likes her very much and wishes she could come more often. Mrs. G.'s daughter who lives in the next town over, has been worried and decided with the urging of her siblings and the doctor to start looking for an assisted living facility for her mother. She found one last week and talked with the Director who said she would be happy to help in whatever way was best. The daughter decided to tell her mother that it was time for her to move, so she can be cared for and be safe. When she told her mother, Mrs. G. cried and said, "This will not happen ever. I plan to stay in this house of loving memories for the remainder of my life."In 3 - 4 pages answer the following questions:
How would you best describe Mrs. G.'s feelings about her life, her family, her traditions, and her future?
Did Mrs. G.'s response to her daughter surprise you? Please explain your answer.
In what way do you believe her culture might be influencing her decision?
If you were Mrs. G.'s daughter what would you say to her that shows you are caring and have compassion for her situation? What nonverbal communication would support that level of communication?
Suppose Mrs. G. stands firm about not leaving her house. What resources and collaborations might be available and helpful so the daughter and other healthcare providers can keep her mother safe and make the most effective decision?

South University Week 5 Nutritional Care Plan Evaluating the Case Data
Nutritional Care Plan For this project, you will identify a case study and then develop a nutritional care plan for the client associated with that case. You should begin working on your project in advance of Week 5 as you construct the details of your case. The completed nutritional care plan for your client is due in Week 10. Please read this entire document to gain understanding of the project scope and those tasks you will complete. This project will take you several weeks to complete. Do not delay beginning the work on this project. PROJECT TASKS TO BE COMPLETED IN WEEK: 5 TASK 1: Choose from one of the following stages: pregnancy, infancy and childhood, adolescence, adulthood, and old age. List the specific nutrition issues affecting individuals in your selected stage. TASK 2: Choose a disease or medical condition that could possibly, or commonly, affect your individual. This could be anything ranging from diabetes to cirrhosis. Using the ABCDs of nutritional assessment, create a case study based on an actual client or patient. You can also make one up. Include the following in your case study: Be sure to include a list of common medications that may be used to treat your patient's condition and identify potential herb/nutrient/drug interactions that may be relevant. Identify your client's cultural background and give clues as to their socioeconomic status and psychosocial variables. For example, your patient may be a senior living alone on a fixed income or might be living in a nursing facility with reputed staff. Here is a general example on how to use the ABCD's of nutrition assessment. Matthew Jones is a 12 year old boy living with his mother and 4 siblings in a rent controlled housing complex. He presents to the local clinic for evaluation for complaints of fatigue, increased thirst as well as polyuria (increased urination) for 1 week. His mother has noticed some weight loss. Height: 5' 10" Weight: 126# Usual Body Weight 138# BMI: 18.1 (This is the A for Anthropometrics) Lab data: Random Glucose 472. Hemoglobin A1C 13. (This is the B for Biochemical Tests) Clinical Assessment: Matthew's lips are dry and cracked. He appears pale. Complains of lethargy, headache, polyuria, polydipsia. (This is the C for Clinical Assessment) Dietary Intake Analysis: Discuss dietary habits. (This is the D for dietary intake)The clinical data indicates a new onset of Type 1 diabetes. At this point go on to discuss medications used to treat, potential problems with food/drug interactions. Address socioeconomic factors which may impact treatment.
