Introduction
Medication non-adherence is becoming more of a problem in today’s society
There are several reasons for this increase from loss of a job, to loss of
healthcare coverage, forgetting to take medication, symptoms or problem has
subsided, or plain old non-adherence.

There are certain illnesses, conditions, and other medical issues that can be
directly related to medication non-adherence. For example, heart attack and
strokes, are more prevalent in African American communities

Change Model Overview
The ACE Star Model of Knowledge Transformation is systematic integration of evidence
into practice that is composed of five stages:
• Knowledge discovery
• Evidence summary
• Translation into practice recommendations
• Integration into practice
• Evaluation (Arudel, 2017).
Nurses use this model as a guide to facilitate how they can attend to their patients while
helping to assist with the problem at hand.

Practice Issue
These are the issues which may be experienced by the medical staff when
dealing with medical non-adherence:






Mandatory overtime to ensure that the patients get medicine on time
Safe patient handling
Safe needles

Workplace violence

Scope of the problem
Primary concern is that patients are becoming more medication non-adherent
and causing more damage over time and making them more suspectibible to
strokes.
Among the highest are black women (4.7%) when compared to white
females (2.9%), and Mexican American women (1.7%) (Beal, 2014).
When they start feeling better they stop taking their medications as
prescribed even though deep down they know the symptoms will return.

Physician assistant
Pharmacist
Possibly the insurance company

Responsibility of Team Members
All these selected members are crucial to the project because the staff
interacts with the patient’s every time they call.

Physicians, nurse practitioner, and physician’s assistant see the patient for
their office visits and discuss medications, how they are working, and any
possible side effects (Mahtani et al, 2011).
