Mini-Lesson Format
Student Name:

Grade: Four

Subject Area: Mathematics
Characteristics/needs of the students with disabilities in the group:
The number of students to focus on is four, and they are all in the fourth grade. One of the
students on focus has intellectual disability, and she reads at the level of a first grade. Learning
disability can be seen in two of the four students, and this is both a girl and a boy. Lastly, there us
a girl with visual impairment and she can not see small writings or drawings that are placed a
distance from her.
Disability Types: Learning disabilities, Intellectual disability, Visual impairment.
Accommodations that need to be made:
I will provide large print in the form of diagrams so as to assist the student with visual
disabilities. This is because large print will help the student to tell the different diagrams as well
as draw them when necessary. Lastly, I will provide more time for both the lessons and test...
