TRIAGE ASSESSMENT FORM: CRISIS INTERVENTION
8 R.A. Myer, R.C. Williams, A.J. Ottens, & A.E. Schmidt

CRISIS EVENT:
Identify and describe briefly the crisis situation:

Suicide. In this case, there is a man who wants to commit suicide as he

is standing at the edge of a bridge. This means that the individual isn’t coping well with the challenges of life. The man
hasn’t jumped yet, so there is time for approach and intervention. This is a very conflicting time for the individual and law
Enforcement because it’s their duty to negotiate with the individual in order to get him down from the edge of the bridge
and help him to seek help.

AFFECTIVE DOMAIN
Identify and describe briefly the affect that is present. (If more than one affect is experienced, rate them in priority order.)
ANGER/HOSTILITY: He possesses a weapon in his hand. This situation doesn’t make anything easier. He seems like he
is angry with life and since he hasn’t fired any shots yet, there is time for intervention

ANXIETY/FEAR:

There is a mixture of emotions and anxiety or fear isn’t clear.

SADNESS/MELANCHOLY:
There is clear sadness in the man’s voice because, in his background story, it seems that he lost his wife to cancer after 54
years of blissful marriage. This reaction is quite expected although the limits may be a little bit dangerous. This is because
it may lead to a disastrous occasion, one that would have been prevented.
