PEDIATRIC CASE STUDY
The purpose of the case study is for you to apply book learning to real-life situations. Never is the care of the child as perfect as the book may sound. You need to apply critical thinking skills as well as the knowledge you are attaining in this class to present the case study.
This case study will be on a child of your choice (not your own child) aged 6 months to 17 years of age. You will provide a comprehensive evaluation of this child to include:
1. Demographics of the child: age, school?, birth order, etc.
2. Social, past and current medical, birth (if under one year), family, medication, surgical history of the child. More may be included if applicable.
3. Vitals, plotted height and weight and head circumference if less than 3 years of age.
4. Family genogram or map going back 2 generations with medical conditions.
5. Current medications, illnesses, diseases/disorders with explanation and references, if any.
6. Psychosocial development based on TWO of the developmental theorists with your reasoning, examples of behavior/language, etc. that supports your choice explaining your choice.
7. Parental concerns: acute or chronic observations with evaluation of how you addressed these: include any referrals to social agencies, schools or specialists, etc. Provide agency names, phones, etc.
8. Nutritional status of the child. Daily intake of foods, drink: does it meet the child’s caloric needs? Patient education is key here. Please list all the areas of this child’s nutritional status that you will address and your plan to address them with the parent. Is the child a picky eater? Toddler? Vegan? Adolescent afraid of weight gain? Young athlete? What are the special considerations for your child regarding nutritional status?
9. Physical exam: Head to Toe. Present this as you would in the assessment part of a SOAP note with proper abbreviations. Don’t forget Tanner staging if the child is over age 8.
10. Using BRIGHT FUTURES reference, discuss key health promotion of this child’s development along with anticipatory guidance for the next 6 months to a year of this child’s life. List at least 4.
11. Discuss any findings you uncovered during this evaluation. Formulate 3-5 nursing diagnoses with proper terms and interventions for this child. How will you evaluate your interventions?
12. Summary and closure to this case study: 2-4 sentences describing this activity from a nursing point of view. “I liked doing this” is not acceptable. Use your critical thinking skills to conclude the case study from a BSN prepared nurse point of view.

Demographic factors: Sonia is a 16-year-old African American girl from Flower Mound,
Texas in the United States. She is in her 10th grade as a student at Flower Mound high school.
Sonia is a second born in a family of three.
Past Medical History (PMH): Sonia has never suffered from any serious chronic conditions
or illnesses such as asthma. However, last year when she was aged 16, she was diagnosed
with HPV Type 1. Therapy and other medications subsequently managed this.
Past Surgical History (PSH): Sonia has never had any surgery in the past.
Current medical condition. Sonia notes that she has developed blisters and itchy sores on
her skin, especially near her genitals.
She says that before the blisters appeared, she
experienced itching, burning, and tingling feelings on her skin. Right now the sores and
blisters are making her experience difficulties and pain while urinating. She also has flue and
rates her genital skin pain at six on a scale of 1 – 10
Allergies: Sonia has allergy reactions towards eggs, NKDA, and PCN.
Immunization: Sonia is current on all her immunizations.
Current Medication: currently, Sonia uses two types of medications to manage her
condition. These are

Ibuprofen, two 500mg tablets in a daily basis for pain management. They are taken in
the morning and evening.

Famciclovir, three 500 mg tablets per day taken orally each after eight hours.
Family History: Sonia lives with her father who has diabetes while her mother has high blood
pressure. Sonia’s grandfather was a healthy man who died in a road accident whereas her
grandmother died aged 76 from breast cancer. Sonia’s two brothers are medically okay
Lifestyle and Nutrition: Sonia states that she is physically active and benefits from a healthy
diet that is provided by her parents. Primarily, she exercises at school where she engages in

physical education and also on some days she rides her bicycle to school. A day cannot pass
by without her taking an apple.
Development theories
The psychosocial, theory by Jean Piaget and Lev Vygotsky states that a child’s
personality develops over eight stages in their lifetime (Silva, et.al, 2013). This development
is primarily influenced by interactions with friends, culture, and family members. In Sonia’s
case, the development of HPV 1 can be associated with her interactions with friends who
might have transferred it to her through nonsexual contact. Also, her culture as an African
American might have influenced the prevalence.
The ecological theory by Uri Bronfenbrenner also explains the occurrences in a child’s
life as being a consequence of natural balance (Pinninti, et.al, 2013). This theory is suitable
in this case because it can help determine the causing factors for Sonia’s condition since she
is still a kid by evaluating the influence of concentric factors on her. The factors have to
include heredity and environment.
Parental concerns: Sonia’s dad was at some point worried that her daughter could have
