Hill Physicians Medical Group is an example of a high performing health care
organization that has utilized different tools to practice medicine efficiently and uniquely. The
case study elaborates how care coordination among a group of independent practitioners can be

physicians are united in an organization that utilizes innovation, technology, and effective
management to realize better outcomes. Further, Hill provides an open-minded approach to care
where solutions to problems are crafted and tested. This evidence-based approach to practice has
led it to become the largest independent practice association in the country. The innovations that
the organization has applied are diverse, and they are either directly replicable or modified to fit

A close analysis of the innovations that the case study discusses as having been applied in
