Question 1.
According to Peña and Valerdi (2014), the requirements of the project have a significant
impact on its variability throughout its life cycle. From this point of view, it is essential that the
causes of requirements volatility are fully understood and under control in order to minimize
engineering effort during the life cycle of the designed product.
I have selected this paper because it has helped me understand the different difficulties
faced when trying to optimize the design of the product not only for its active live, but during its
whole life cycle. In this regard, so far I had thought that it was important that the product met the
technical specifications when it was being used, but was not aware of how important it was that
these technical specifications were matched as well once the product had been discarded.

Question 2.
I think that systems engineering provides an invaluable tool to use the theoretical
concepts applied in class to solve a real-life problem. From this point of view, I would like to
meet Fellow Duane Hybertson, since he had contributed so much at promoting the
interrelationship existing between theory and practice in systems engineering. In this regard, he
had been awarded the Fellow Award in 2014 for his significant contributions working towards
bringing together systems science theory and system engineering practice.

Question 3.
The system design should cover all the steps in the life cycle of the product, including the
supply of raw materials, the manufacturing process, the period during which the product is being
used, and the stage after it has been finally disposed to the environment once it is no longer
useful for the company.

Question 4.
Requirements must serve to identify the necessary design parameters of the product so
that it matches the technical specifications. If there is a conflict between the product’s
requirements and the TPM, then the design project needs to be reconsidered and the
requirements need to be reformulated.

Question 1.
Examples of hazards include an earthquake, a thunderstorm or an airborne disease like
Legionella. They are hazards because they can cause us physical, economic or health harm and
they can’t easily be avoided.
In contrast, dangers are as well able of causing harm, but their effect can be easily
avoided or warned through the use of danger signs. Taking this into account, some examples of
danger would be a part of an instrument that gets hot, the pieces of broken glass and an extension
cord lying on the ground.
Finally, a risk represents the chance that something will cause harm. Taking the above
examples into account, the possible risks would be to get injured by falling down with the
extension cord, to get burned with the hot parts of an instrument or to get sick from the airborn
disease.

Question 2.
The following picture represents the diagram of an ergonomic hand tool used to cut
wires. It has been designed considering the principles of the fundamental ergonomics model.

Comparison of the different industries
The incidence rate is highest for the transportation and warehousing industry in 2014.
During this year, the incidence rate of non-fatal occupational injuries and illnesses was of 7.5
cases for every 100 employees. However, the incidence rate of this industry in the rest of years is

similar to that of the agriculture, fishing and forestry industry during the 2010-2015 period,
where an incidence rate of 4.5 – 5.7 cases per 100 employees is observed.
Taking this into account, the agriculture, fishing and forestry industry and the
transportation and warehousing industry would be the ones having the highest incidence rates of
non-fatal occupational injuries and illnesses from those industries analyzed. On the other hand,
the mining industry has the lowest incidence rate of non-fatal occupational injuries and illnesses,
with only 1.4-2.3 cases per 100 employees.
On the other hand, it is interesting to observe that the transportation and warehousing
industry presents the highest number of total days lost from work and cases involving days away
from work. However, it is not the industry having the highest number of cases involving days
with restricted activity at work. Even while further research would be necessary to clarify this
issue, it would seem that the transportation and warehousing has a higher severity of injuries and
illnesses that imply that employees are absent from work rather than just having a restricted
activity.
Taking the previous analysis into account, the recommendations for the different
industries would be:


To verify the security standards applied in the agriculture industry to check if any
measure is failing so that the incidence rate of non-fatal injuries and illnesses seems
to increase over time



To check what has caused the higher incidence rate of illnesses and injuries in the
transportation and warehousing industry in 2014, since this data does not seem to be
related to a higher number of cases involving absence from work being reported or a
higher severity of the reported cases.
