Electronic Health Records are electronic versions of
the medical history of patients that are maintained
by hospitals and other care givers and contain all
relevant clinical data including administrative
records, demographics, medical problems,
progress notes, vital signs, prescribed medications,
laboratory results and radiology results of patients.
(Walker et al, 2005).

The Electronic Health Records system provides an
automated platform where information can be
accessed.
The system acts to improve the care for patients
by making the health information for the patients
readily available. (World Health Organization,
2006).
Decreasing the duplication of medical tests.
Reducing the treatment time for patients and
informs the patients of the better decisions they
should make.
The system also helps to reduce the medical
error by making the medical records more
accurate and clear.

Description of the issue

There are some issues that are concerned
with the electronic health records. They include:
 Security
 Privacy
 Confidentiality and
 Data inaccuracies (DeVore & Kinsey, 2010).

Patient’s information should be secured and
only accessed by authorized personnel.
However cases arise that hackers and other
unauthorized persons get access to the
patients’ information illegally.

In Howard University Hospital, a technician
was prosecuted for using her capacity in the
hospital to illegally gain and sell patients’
information to criminals. (Mennerat et al,
2002)

According to the Health Insurance Portability and
Accountability Act (HIPAA), patients’ data that is
recorded and stored in the electronic systems should
always be confidential and should not be disclosed
without the consent of the patient or only when it is
necessary.
The government may need the patients’ general data
to generate specific reports on specific issues in the
health field.
When a patient is unable to provide necessary
information because of old age, the information will
be provided in the presence of a legal representative.
According to studies that have been conducted, some
healthcare givers and institution have violated the act
and have been found to disclose patient’s
information illegally.

A case was reported that a nurse in the
Howard University shared personal
information of a patient that suffered from
HIV/AIDS. The information caused the patient
to be stigmatized by members of the
community in which they were living in. The
nurse was arrested and prosecuted.
(Mennerat et al, 2002)

The information that is stored electronically
about patients should be: accurate, relevant
and specific for each patient.
There are cases where the information stored
for some patients are not accurate and are
not specific.
During data entry or transfer, the loss of data
may happen and this may cause presence of
inaccurate data.

The Implementation of the electronic heath
records by hospitals and other health care
providers is very costly and requires specific
sets of skills which are expensive for most
health institutions. (Sinha et al, 2013)

Relevance and Significance of the Issue

Security Issues

For individuals, lack of data security affect them directly as they
may be victims of cyber crime. Manipulation and deletion of medical
data due to poor security may affect the health of individuals as it
may lead to wrong diagnosis and wrong treatment.

Privacy and Confidentiality Issues

When the privacy and confidentiality of individuals are violated and
their information revealed illegally, they may be harmed either
physically or mentally by the people who have access to their
information.

Data Inaccuracy issues

Addressing the data inaccuracy issues is important as inaccuracy
issues affect individuals as their medical conditions may be
misdiagnosed leading to serious complications or fatalities.

Cost of Implementation issues

The cost of Implementing the EHR affect individuals as they
result in the implementation of systems that are not up to
standard and will result to problems in the future for individuals
that will interact with the electronic platform.

Security Issues

Groups such as patients suffering from a common condition may be
affected when their records are tampered with. When records are
deleted or illegally modified, the medication process may be
disrupted.

Privacy and Confidentiality Issues

when the confidentiality and privacy rights are violated for a certain
group that is receiving medical attention, conflict may arise and
some individuals may withdraw from the group for fear of being
victimized.

Data Inaccuracy issues

When group data is inaccurately entered, this may affect the
dynamics of the group resulting to conflict and misunderstanding in
the group

Cost of Implementation issues

The cost of implementing the EHR affects the quality of the overall
system thus affects the groups that will interact with the system.

Security Issues

The society loses trust in the country’s health sector when they
notice that their records in hospitals are not secure.

Privacy and Confidentiality Issues

The society takes confidentiality issues very seriously and when they
are not confident that their information is kept private, they may not
access certain health facilities even when they need the services.

Data Inaccuracy issues

Members of the society will not have confidence with a system that
does not take care of their records and that causes death because of
inaccuracy.

Cost of Implementation issues

When the cost of implementing systems is high, the implemented
system will not be accurate and secure, this will raise the attention
of the society and will end up losing confidence in the system.

Healthcare in the United States
