HA 518 Park University Unit 2 Beliefs Values and Health Discussion
Unit 2: DiscussionBeliefs, Values, and HealthIntroduction Over the past decade, regulatory changes have created new opportunities for cross-sector collaboration around chronic disease prevention. The passage of the Affordable Care Act (ACA) in 2010 provided a legal foundation for widespread reform od the healthcare system around such goals as expanding health insurance converage, strengthening primary care access, incentivizing prevention, and increasing integration between healthcare and public health. Requirements for nonprofit hospitals to conduct periodic community health needs assessments, to engage public health partners in community health planning, and to report their contributions to their communities all contribute to an environment conducive to collaboration. In this LearnScape, the student is functioning as an external consultant working with the Director of Marketing, at Bright Road Healthcare System, to find ways to improve the functionality and utility of the system’s patient health record (PHR) application. Not only does the PHR engage patients in their own wellness and a method of retaining loyal clients, can also be critical for the success of collaboration in integrated care delivery. There is a growing focus on population health management. Population health management focuses on promoting wellness through an emphasis on prevention which in turn controls healthcare costs. If the population is doing a better job managing their health status, then they are less likely to access healthcare only when they experience an acute event. This should result in a reduction in the emergency room visits, which is perhaps one of the most expensive points of service in the U. S. healthcare delivery system. In order to make a comprehensive population health management initiative successful, it requires a high degree of collaboration between chief stakeholders (public health, hospitals, academic medical centers, insurance/managed care organizations, patient-centered medical homes, accountable care organizations, and, of course, the population). There is also a need to have adequate information systems to support such a comprehensive wellness promotion strategy. The personal health record (PHR) is one such system that is designed to permit patients to compile and maintain information about their health. In addition, clients retain control over how the health information is accessed, used, and disclosed to third parties. While the PHR is similar to an official medical record, they are not the same. The medical record contains information provided by healthcare personnel. The PHR and patient portals are examples of information systems that are designed to increase population health management, foster patient engagement, and promote repeat business to healthcare organizations and providers. Unit Learning Outcomes ULO #1: Analyze relevant trends within a community, and then leverage this information to promote population health management programming. (CLO 3 and 6)ULO #2: Examine how information systems and technology support population health management initiatives. Based on the information provided in the scenario and an outside review of the available literature, taking into the account the upcoming integration with primary care practices prepare a formal recommendation to the Director of Marketing on steps the organization should consider undertaking to improve the functionality and utility of its personal health record (PHR) system. When formulating the recommendation, it is important to take into consideration the industry trend towards promoting population health management and the need for effective integrated information systems to support these initiatives. The recommendation needs to be well supported, logically presented, and thoroughly vetted. Initial PostingsInitial PostingThe initial posting should be prepared as a Microsoft™ Word document, and then attached to the unit discussion thread. There is no minimum or maximum in terms of the word count; however, the response should explicitly address all required components of this discussion assignment. The document should be prepared as a practical business deliverable (single-spaced) and reflect higher-level cognitive processing (analysis, synthesis, and evaluation). Support the initial posting with at least 3 references of credible or peer-reviewed sources. The in-text citations and the list of references should be consistent with the APA writing style (7th ed.). To earn the maximum number of points for the initial posting, the overall quality of the writing should be consistent with that expected of a graduate student and industry professional. Due DatesOnline Students: The initial response is due by Wednesday at 11:59 p.m. (CT). Respond to a minimum of two peers’ initial postings. The peer responses are due by 11:59 p.m. (CT), on Sunday. Blended Students: The initial response is to be posted to the unit discussion thread at least five (5) hours PRIOR to the scheduled class session. Respond to a minimum of two peers’ initial postings. The peer responses are due by 11:59 p.m. (CT), on Sunday.
