GCGU Health Informatics Regulatory Issues and Standards Discussion
Health Informatics Regulatory Issues and Standards Objectives: Describe processes and structures for regulating health information technology (health IT). Explain how Meaningful Use criteria are driving the development of the health record and the use of information within and across health care organizations. Examine how the adoption of standardized vocabularies and terminologies can assist in the implementation of Meaningful Use criteria and development of electronic health records (EHRs). Describe the importance of data exchange across settings and the role of standardized terminologies in the development process of data exchange. ***There are four (4) discussion questions from two different topics for this assignment instead of the usual two discussions and one assignment. Therefore, with the above knowledge, answer the following questions in a discussion format. Ensure all questions are responded to appropriately with references attached. Discussion 1 Discuss the primary regulations related to patient privacy and security that affect the how information is stored, retrieved, and utilized in the health care environment. What are the major challenges facing health care organizations as they work to implement regulations and standards, including meaningful use criteria. Discussion 2 Discuss both the value and challenges of implementing standard vocabularies and terminologies in the storage and use of health care data. How do these challenges impact informatics professionals and the work they do in health care settings? Resources Read Chapters 22, 25, 27, and 28 in Health Informatics: An Interprofessional Approach. Read "Not What the Doctor Ordered: Security Concerns in Light of Evolving Health Technologies: Health Care Professionals Must Stay Current on How Best to Protect Patient Data," by Lanterman, from Journal of Health Care Compliance (2015). Consumer Applications Objectives: Evaluate mobile technologies that can enhance patient care. Discuss trends in consumer health related-applications and consumer informatics that impact the storage, retrieval, and use of health care data and information. Examine the challenges of differentiating between data provided by consumer applications and devices versus clinical data from the EHR. Explain the impact of telemedicine on consumers, providers, health care centers, and the documentation of health care data. Discussion 3 Discuss an example of a mobile device or social or health-related application you use in your everyday life that stores personal health data and could potentially benefit your health. How could the technology or application be improved and utilized in the health care setting to provide a more detailed or holistic picture of a person's health status to providers? Discussion 4 What are the primary challenges with integrating data from personal devices and applications with data within an EHR? Do you think most consumers are comfortable with sharing their personal data in the clinical environment? Supposing the sharing of such information is beneficial for consumers' health, what can be done to help consumers overcome their reticence? Resources Read Chapters 8 and 12-14 in Health Informatics: An Interprofessional Approach. Harvard University Hospital Readmission Rate Case Study
Case Study Scenario The hospital readmission rate is often considered an indicator of an undesirable patient outcome. You quality improvement team is located at a metropolitan hospital that serves a large population. The quality improvement team is conducting a study pertaining to readmissions, smoking and number of drugs administered to admitted patients with congestive heart failure (CHF). The team is interested in reducing the number of readmissions among patients discharged with a principal diagnosis of CHF. The team believes that the high admission rate is due to the difficulty that these patients have in controlling the number of drugs that they typically take. The team believes that by improving patient/family education regarding drug administration, the readmission rate could be reduced. Thus, they have developed the screen “CHF patients taking three or more drugs” to identify these patients before discharge. To evaluate the effectiveness of the measure, the team conducts a study on all CHF patients discharged the previous year. The results are shown below: CHF patients Number of Drugs Administered Readmitted Not readmitted Total ≥3 drugs 200 40 240 <3 drugs 100 900 1000 Total 300 940 1240 Your team needs to decide how to measure the effectiveness of this measure. You think the best way would be to compute the values of the sensitivity, specificity and predictive value. Show your work and provide an interpretation of these measures. On the basis of your results, is this an effective measure? Why or why not?Once you assess the effectiveness of this measure, and your team feels confident that this is an appropriate measure to use, the Health Administrator of the Unit asks your team to examine the odds of patients being readmitted for those who were administered at least 3 drugs. Calculate the odds ratio and explain this finding to your administrator. Next, your team is examining the number of cigarettes smoked per day in this sample and sex-specific death rate per 1000 per year among CHF patients admitted to the hospital. The following table shows the results that you obtained.Your team would like to calculate the attributable risk or attributable proportion of CHF death rates due to smoking 1-14 and 15-24 cigarettes per day separately for males and females. Do you notice any differences within and between these groups in death rates due to CHF based on the different number of cigarettes smoked per day? What can the team conclude about the death rate due to CHF for males and females separately when zero cigarettes per day are smoked? How do they compare?Obtain a regression equation for the data shown above that predicts readmissions based on the number of drugs administered. How is the number of drugs administered associated with readmissions for these patients? Cigarettes/Day Death Rate/1000/Year due to CHF Males Death Rate/1000/Year due to CHF Females 0 0.01 0.008 1-14 0.27 0.11 15-24 1.23 1.15 25+ 2.00 1.50 Your quality improvement Team is revisiting the following patient readmissions and would like to predict the number of readmissions based on the number of drugs administered. CHF patients Number of Drugs Administered Readmissions 1 100 2 100 3 200 4 400 5 500 6 550 7 600 8 630 9 660

TCC Diverse Communities Common Concerns Assessing Health Care Quality For Minority Discussion
A growing, profitable internal medicine practice of 10 physicians, 2 physician assistants, a physical therapist, and a massage therapist is located in a city of 150,000 people. The clinical personnel are mostly from the majority group in the area and consider themselves open-minded, altruistic, and culturally aware. The administrative support staff is representative of the makeup of the patient population—mostly minority groups in the local community. The patient population is growing and becoming more diverse from those newly arrived in the city. The practice is currently facing several challenges. QuestIons 1. The practice manager wants to have the local hospital expert on cultural competency come talk to the staff. The administrative support staff does not feel that they need training. How should the practice manager proceed? 2. The practice needs to hire a new administrative assistant to work with the front desk staff. The practice wishes to hire someone from the local minority community rather than an equally qualified individual from another state. How should the practice manager proceed? Justify the rationale for this recommendation. 3. With the recent growth, the practice is considering relocating its office to another, larger office facility. Several options are being discussed. Should they relocate to a more affluent area of the city that is less diverse? Or should they stay in the same neighborhood, even if it is less profitable? What are the pros and cons of staying in this neighborhood versus moving to another area? If the rent is higher in the more diverse area, is that enough to justify not moving there? Or is diversity responsiveness a “cost of doing business?” How should the practice manager proceed?4. The practice wants to maintain its good standing in the community. What initiatives could it undertake to remain connected to its local patient population? 5. The practice manager wants to complete a strategic plan as a guiding framework for the organization for the next five years. From whom should she get input to help shape this plan? Should cultural competency initiatives be included in this plan? The length of the case analysis should be approximately 1,000 words (excluding the reference list at the end of the paper). The paper should also apply and cite at least three external references above and beyond the textbook. APA format is required.
